Our goal is to empower our clients by providing comprehensive support that addresses their holistic needs and enhances their ability to thrive in independent living environments.

What we do:

Assess the needs of our clients, including their families where relevant, is a foundational aspect of our services. We specialize in coordinating, arranging, and advocating for access to diverse programs and services tailored to meet each individual's unique needs. Our comprehensive case management approach encompasses various activities that foster independence and success in transitioning to independent living.

Examples of case management activities we engage in include:

• Assist in navigating housing options.

• Collaborate with local landlords to secure suitable housing for clients, including facilitating connections to financial assistance programs.

• Offer transportation services for clients to attend rental showings.

• Assist with name changes through the Social Security Administration.

• Arrange transportation for clients to attend therapy appointments.

• Facilitate food pantry pickups and deliveries for clients.

• Collect donations from local thrift stores in Cloquet and Moose Lake to fulfill clients' basic needs, such as clothing, school supplies, personal hygiene products, and household essentials.

• Distribute resources to clients, including gas cards.

• Partner with a donation program during the 2023 holiday season to provide clients and their families with multiple gifts, with delivery options for those unable to visit our office for pickup.
